Brazil Approves Novo Nordisk’s Wegovy Obesity Pill

The approval opens Brazil’s obesity market to a daily oral GLP-1 treatment after clinical trials showed average weight loss exceeding 13%

Wegovy

By Brazil Stock Guide – Brazil’s health regulator Anvisa approved an oral version of Novo Nordisk’s (NYSE: NVO; CPH: NOVO-B) blockbuster obesity drug Wegovy on Wednesday, November 4, marking the country’s first authorization of a GLP-1 pill specifically for weight management.

The decision allows the Danish pharmaceutical company to expand its obesity treatment portfolio in Latin America’s largest economy, where Wegovy had previously been available only as a subcutaneous injection, according to Agência Brasil. The new formulation offers a once-daily alternative for adults with obesity or overweight accompanied by weight-related medical conditions.

The approval is supported by results from the Phase 3 OASIS 4 clinical trial, which demonstrated average weight loss exceeding 13% over 64 weeks among participants receiving oral semaglutide at a daily maintenance dose of 25 milligrams. The treatment significantly outperformed placebo and improved several cardiometabolic health indicators.

For Novo Nordisk, the regulatory milestone broadens the commercial potential of its semaglutide franchise by offering a non-injectable treatment option in Brazil. The oral formulation could appeal to patients who prefer tablets over injections, although adoption will depend on pricing, availability and medical prescribing decisions.

Clinical Results Support Approval

Anvisa based its decision primarily on the OASIS 4 trial, which evaluated the efficacy of oral semaglutide in adults with obesity or overweight and associated medical conditions.

Participants receiving the 25-milligram treatment achieved average weight reductions exceeding 13% during the 64-week study period, substantially outperforming those receiving placebo.

The trial also demonstrated improvements in several markers associated with cardiovascular and metabolic health, reinforcing the potential benefits of weight management beyond body mass reduction.

Semaglutide belongs to the glucagon-like peptide-1 receptor agonist class, commonly known as GLP-1 drugs. These treatments mimic a naturally occurring hormone involved in appetite regulation, helping patients feel full and reduce food consumption.

“Semaglutide acts on the regulation of appetite and food intake, promoting body weight reduction and improvements in metabolic parameters associated with obesity,” Anvisa said.

The results provide clinical support for the use of oral semaglutide as an additional treatment option for patients who meet the approved eligibility criteria.

Daily Dosing and Treatment Requirements

Unlike the injectable version of Wegovy, which is administered weekly, the oral formulation requires daily dosing.

The approved treatment follows a gradual escalation schedule, beginning at 1.5 milligrams per day before increasing to 4 milligrams and 9 milligrams. The recommended maintenance dose is 25 milligrams daily.

Patients must take the tablet on an empty stomach with a small amount of water and wait at least 30 minutes before consuming food, beverages or other medications.

The formulation is intended for adults with obesity, defined as a body mass index of at least 30, or overweight patients with a BMI of at least 27 and one or more weight-related medical conditions.

Treatment must be accompanied by a reduced-calorie diet and increased physical activity.

Anvisa also authorized clinically appropriate transitions between oral and injectable formulations, subject to the prescribing guidance outlined in the approved product information.

Commercial Implications for Novo Nordisk

The regulatory clearance strengthens Novo Nordisk’s portfolio in a market where demand for pharmaceutical obesity treatments has created opportunities for drugmakers developing GLP-1 therapies.

The availability of an oral formulation gives the company another route to reach patients, particularly those who have difficulty with injectable treatments or prefer a daily pill.

Novo Nordisk already markets injectable Wegovy for obesity and other semaglutide-based products, including Ozempic and the oral diabetes medication Rybelsus.

The new Brazilian authorization specifically covers weight management, expanding the use of oral semaglutide beyond its existing diabetes treatment applications.

Brazilian pricing for the new Wegovy formulation has not yet been established. The approval of the product’s registration does not mean it is immediately available in pharmacies, as commercial distribution and pricing arrangements remain separate steps.

The company expects commercialization in Brazil to begin during the first half of 2027, according to CNBC’s Brazilian affiliate Times Brasil.

Obesity Treatment and Long-Term Health Risks

The approval comes as pharmaceutical companies expand treatment options for obesity, a chronic and progressive condition associated with multiple metabolic and cardiovascular complications.

Obesity increases the risk of type 2 diabetes, hypertension, abnormal cholesterol levels, cardiovascular disease and obstructive sleep apnea.

Anvisa considers the oral formulation an additional therapeutic option that may provide greater flexibility for healthcare professionals and patients managing the condition.

The regulator’s decision establishes another approved use for oral GLP-1 therapy in Brazil, supported by clinical evidence of sustained weight reduction and improvements in metabolic health.
